Lack of ‘real’ time communication in the organisation and delivery of home support services affects the continuity and quality of care given in the home. There are multiple and unnecessary stages in the chain of communication between all those involved in the delivery of person centred home support services such as carers, nurses, private care agencies, managers, physiotherapists, doctors, social workers. How could technology support the sharing of day to day information instantly, safely and securely around older persons especially those with complex care needs?

Anonymous user 2 0 @Jennshare123 Well Jenn, I think with a log, you could have both. Daily log of people contacting client, in person, phone etc. Also, themes could be itemised under headings, egJ:meals, mobility , personal care, etc: That way everyone can link in under topics and also there is a chronological record of attendences. (Good for reminding of days events).
The facilitator has to start the meeting.
